Reservation Policy:

 

Please carefully review the following terms and conditions before proceeding with your reservation. The signee of this reservation form assumes responsibility for all individuals included on the form and should ensure their awareness of these terms and conditions.

 

The nightly rate covers 3 meals per day, snacks, beverages, 24/7 recovery assistance, wifi, cable, and laundry services. Our team consist of CNA's and HHA's, with a nurse practitioner available for on-call support. We offer transportation services through a third-party company at a cost of $320, which includes airport pick-up for pre-op, surgery drop-off and pick-up, 1 post-op appointment, and airport drop-off. Additional rides outside of this package will incur extra charges depending on the location, and pricing is subject to change. When booking with GLRC Service LLC, all transportation services must be arranged through Miami SX Transports. For safety reasons, we do not disclose our address. However, once you arrive, you can share your location with your family members. To secure your reservation, a non-refundable/non-transferable deposit of $350 per person is required (the deposit will be deducted from the total balance). The remaining balance must be settled TWO DAYS PRIOR to your arrival via Zelle or TWO WEEKS PRIOR to your arrival via invoice credit/debit. Failure to fulfill the remaining balance will result in cancellation. We do not accept cash payments. Please note that once surgery is complete, there are no refunds. By making a deposit to Lotus Miami, you agree to the following terms and conditions. Due to Covid-19, we do not allow guests.

Cancellation Policy:

 

If you need to cancel or reschedule your reservation, you may do so ONCE under the following terms:

 

- Deposits are valid for up to one year, or the reservation can be rescheduled once.

- To avoid a penalty fee of the deposit, you must reschedule your reservation at least 30 days or more prior to the arrival date.

- Any changes to the reservation must be made at least 30 days prior to the arrival date.

- All deposits are non-refundable and non-transferable.

- If surgery is canceled on the day of surgery or on the pre-op day for any reason, a new deposit will be required to reschedule.

- If surgery is canceled due to smoking, high BMI, pregnancy, or any reason on the client's behalf, there will be a 40% fee based on the remaining balance. If you wish to reschedule with a new reservation, a deposit will be required. This policy applies to third-party services as well.

- Payments are only refundable if the procedure is canceled by the surgeon or the surgery facility due to failure to meet medical clearance, with documented proof of cancellation approved by the GLRC Service LLC management team. This policy applies to third-party services as well.

 

Return Policy:

 

We regret to inform you that we are unable to accept returns or exchanges on undergarments, absorbent materials, or supplies. Please refer to our sizing chart when placing an order to ensure the correct size, as all sales are final and non-refundable.
